Starbucks giftcard now available as incentive to participate in survey regarding genetic services for early-onset breast cancer survivors in Georgia

Below is a survey to assess the referral status and utilization of genetic services by early-onset breast cancer survivors in the state of Georgia. This survey should identify whether genetic services are being appropriately administered or are being forgotten amidst diagnosis and treatment. The results of this survey will be used to shape educational outreach to Georgia providers and improve access to genetic services for women in the state of Georgia.

The survey is online and will take approximately 15-20 minutes to complete. Individuals who complete the survey will have the opportunity to win a Starbucks giftcard. To be eligible for the giftcard, please re-take the survey even if you've previously completed it.
