Opportunity to Participate in Statewide Survivorship Needs Assessment

Are you or someone you know a cancer survivor? If so, you are the expert, and we want to hear from you!

Georgia CORE is conducting a survey of cancer survivors throughout the state of Georgia. The purpose of the survey is to assess the needs of survivors across all aspects of their life in order to better inform hospitals, organizations, agencies and other interested groups about the concerns of survivors. Your support and participation is critical in helping us reach a diverse group of cancer survivors in Georgia!
The survey consists of multiple choice and short answer questions and is estimated to take no longer than 30 minutes. The survey is available online here or you can copy/paste the following link into your browser: https://redcap.emory.edu/surveys/?s=73wVEWEkgQ.

Survivors’ names and treatment institutions will be recorded so that Commission on Cancer hospitals can receive quality of life accrual credit for their survivors’ participation. All responses to the survey will be kept confidential and participation is voluntary. Individual information will not be used in reports or documents.

To be eligible to take the survey, individuals must:

1) Be a cancer survivor (According to the National Cancer Institute, cancer survivorship starts from the point of diagnosis and extends to the end of one’s life.) 2) Be 18 years of age or older. 3) Live in Georgia and/or have received their cancer treatment in Georgia.
